Position Overview:
Medication dispensing: Pharmacists verify prescriptions and dispense appropriate medications to patients based on their medical condition and dosage requirements. They ensure accuracy in medication selection, dosage, labeling, and packaging.
Medication management: Pharmacists are responsible for maintaining an inventory of medications in the hospital pharmacy, monitoring expiration dates, and ensuring proper storage conditions. They collaborate with other healthcare professionals to establish and maintain formularies, which are lists of approved medications for use within the hospital.
Patient counseling: Pharmacists provide information and counseling to patients about their medications, including dosage instructions, potential side effects, drug interactions, and precautions. They educate patients on the proper use of devices such as inhalers, insulin pens, or injectable medications.
Medication reconciliation: Pharmacists play a vital role in medication reconciliation, which involves reviewing a patient's medication history and ensuring accurate and up-to-date medication lists. They collaborate with healthcare teams to identify and resolve any discrepancies or potential medication-related problems.
Clinical pharmacy services: Pharmacists actively participate in patient care rounds and provide medication-related recommendations to physicians, nurses, and other healthcare professionals. They assess patients' medication therapy, monitor drug therapy outcomes, and make necessary adjustments to optimize therapeutic outcomes while minimizing adverse effects.
